Episode 12 begins by showing Deku nearing the Sky Coffin. Meanwhile, at the Sky Coffin, the heroes are shocked by what happened to Bakugo. Shigaraki blames Aizawa for deploying young heroes to the Frontline. Enraged by what happened, Mirko charges at the villain but gets caught.

Elsewhere, at the Kamino ward, Dabi makes his return to the scene. He had survived his brother’s last skill by copying his technique, unveiling the bright blue X on his chest. Skeptic also made an appearance as he was hacking the sky coffin. Meanwhile, at Gunga Villa, AFO was slowly reviving thanks to a serum by Dr Garaki that mimicked Eri’s Quirk. With the serum, he was able to regain his youthful body. However, the villain revealed that using this would eventually kill him. He promised to at least free Shigaraki so he could continue his work.

Meanwhile, in the United States, the president blames Timothy for putting a target on their back. The president reveals that several countries were competing to win Shigaraki’s favor. However, Timothy stands against it, reminding the president of what Star and Stripe had been fighting for.

Back at the Sky Coffin, despite Best Jeanist’s attempts, Bakugos’s condition remains critical. However, Edgeshot decides to sacrifice himself to fix the Hero’s heart. As this happens, the remaining heroes try to shift Shigaraki’s attention away. Mirkos’ lethal attack forces the villain’s body to take on a form suitable for combat. Mirio decides to do all he can to stop the villain. The episode ends with Midoriya finally arriving.
