At the conclusion of The Boys Season 4, Episode 3, we see a bloody metal object near Sage. It’s hard to identify what it is from the angle, and it leaves us wondering whose blood is on the metal tool and what it’s used for. Fortunately, after spending a bit of time going frame-by-frame, we’ve identified this mysterious metal implement.
What is the bloody metal tool near Sage at the end of The Boys S04E03?
After close examination, we determined that the bloody metal tool at the end of The Boy Season 4, Episode 3 is an Orbitoclast. This obscure implement was developed in 1948 to perform lobotomies. However, the episode doesn’t reveal what it’s doing in Sage’s room.
It’s possible Sage lobotomized someone with the Orbitoclast, but that’s not necessarily its only use. It’s just a long, thin metal rod with a point at one end and a small handle at the other. It could be an impromptu weapon or a lever to pry something. Obviously, the blood leads us to believe it was used for dubious purposes. Still, perhaps episode 4 will bring us a twist.
Sage isn’t known for getting her hands dirty at this point, so this might be the first indicator of physical violence involving her. Unfortunately, the episode ends before we can get any more clues.
When used for a transorbital lobotomy, the Orbitoclast is wedged beside the eye, and a small hammer is used to break through the thin layer of bone and into the frontal lobe. It would then be used to cut through the brain in two places before being removed. The process would then be repeated on the other side.
While it was touted as a cure-all for mental illness in the 1940s through the 1960s, it’s now considered to be one of the most barbaric procedures that were ever in widespread practice. Needless to say, Sage is likely up to some freaky antics, which will hopefully be revealed in episode 4 of The Boys Season 4.
