Suddenly, the show cuts back to Richard sitting back in his chair with no injuries. Though the pain was imagined, it’s clear that he was shaken by the ordeal.
As he makes his way upstairs, a smoke detector can be heard going off. The show then fades to a shot of the Sackler name with the “S” burned and smoking.

Yes, Glen Kryger dies at the end of Painkiller.
After achieving 30 days of sobriety, Glen unfortunately stumbles upon some OxyContin. He then consumes the drug, ending his streak of sobriety and, ultimately, his life.
Over the emergency services’ radios, it’s clear that Glen has died from an overdose.
